Alternatively, you also have the option of requesting a copy of the report at the precinct where the report was filed. You need to fill out a download and complete two copies of the “Request for Copy of Collision Record” form by either dropping them off in person or sending them via mail. The request must be made within 30 days from when the report was filed. 

The collision reports will only be held by the relevant police precinct for 30 days before they are forwarded to the New York State Commissioner of Motor Vehicles. Therefore, if you are hoping to access a copy of the accident report after the initial 30 days, you need to submit your request to the New York DMV through form MV-198C.
